By submitting this form, I agree that I have filed proof of possession of general liability insurance in the minimum amount of $250,000 for both bodily injury and property damage, and will pay the $25 license fee as required by Chapter 140.11 of the City Code of Story City, Iowa.
Once licensed is approved, the applicant is permitted to engage in the business of pruning, treating, or removing trees or otherwise performing the work of an arborist in the City of Story City for the current calendar year of the application, unless sooner revoked by the City of Story City, Iowa.
This permit allows use of the north tree dump site for disposal of trees removed from within the city limits of Story City only. The key to the gate can be picked up at City Hall, and it can not be shared. No materials can be removed from the tree dump site.
